The Pamela Mam Health Centre provides a range of disability services and supports for Aboriginal and Torres Strait Islander people and their immediate families.

Our deadly team understands that your disability needs are unique to you. We are committed to providing individualised and culturally safe support by tailoring your disability journey to connect you with the most appropriate funding supports and services. We’re here to walk alongside you every step of the way and offer disability support for Mob.

Navigating and accessing the NDIS and the QCSS

For Mob living in South East Queensland, we offer a unique Disability Access Pathway tailored to your needs. Our team works with you to explore disability funding options that best suit your individual circumstances. We help determine your eligibility for both the Queensland Community Support Scheme (QCSS) and the National Disability Insurance Scheme (NDIS), supporting you through the entire application process. This includes working closely with your treating health professionals to gather the necessary evidence, assisting with planning, and linking you to the right service providers. 

Who can access our services?

Access pathways and support are free for Aboriginal and Torres Strait Islander people aged 0 to 65, and their families, who are living with: 

  • a disability 
  • a chronic illness or complex mental health condition 
  • circumstances that affect their ability to live independently in the community. 

If you meet the access requirements, you may be eligible for a NDIS plan and be able to access a range of services, including: 

  • allied health 
  • community access supports 
  • in-home supports 
  • support coordination

Support Coordination

We can support you in understanding your NDIS plan and connect you to the services and programs available, or those that need to be identified in your plan. 

Allied Health

We can connect you to our network of allied health professionals to support your health and wellbeing. Our team includes occupational therapists, speech therapists, physiotherapists, podiatrists, dietitians, social workers, exercise physiologists, therapy assistants, and more under your NDIS plan.  

In Home Supports

We can connect Mob living in the Goodna region with services to provide: 

  • housework – including preparing meals, cleaning and household chores 
  • personal care – such as showering and getting dressed. 

Community Access Supports

We can connect Mob living in the Goodna region with services to provide support with shopping, transport, health appointments and attending social or community events.
